Medical - AETNA

Comprehensive medical coverage, including preventive care and prescription drug plan

Choice of two PPO options.

Vision - AETNA

Coverage for preventive, diagnostic and orthodontic care

Basic Accidental & Life Insurance – Reliance Standard Life
LOGIX provides each employee $20,000 coverage for basic life & AD&D insurance
Voluntary Life Insurance –Reliance Standard Life
Opportunity for employee to purchase additional life insurance up to $500,000 $80,000 guaranteed issue (Under age 60)
$10,000 guaranteed issue (Age 60-70)
Voluntary Dependent Life Insurance – Reliance Standard Life
Opportunity for employee to purchase additional life insurance for spouse and/or child(ren):
  • $30,000 for Spouse under age 60 (if employee purchases at least $50,000 supplemental coverage)
  • $10,000 each child (Age 6 mos-25 years)

Employee paid coverage provides 60% of weekly base pay to a maximum of eleven (11) weeks if an injury or illness prevents an employee from working

LOGIX provided plan which provides basic LTD coverage at 60% after 11 weeks of STD
